Description:HVCS a Division of Cornerstone Family Healthcare is
actively recruiting for a Emergency Financial Services Program
Coordinator to join our growing team in Hawthorne, NY.RATE OF
PAY/SALARY: $23.08 per hourWORK LOCATION(S): Hawthorne, NYJOB
SUMMARY:The Emergency Financial Services (EFS) provides limited
one-time or short-term payments to assist individuals with HIV/AIDS
with an emergent need for paying for essential utilities, food,
transportation and medication.The Program Coordinator is
responsible for program management, including monitoring, reporting
and quality assurance activities; coordination with the staff
accountant to ensure accurate fiscal monitoring and expenditure
planning; activities that include conducting brief intake
assessments, documenting necessity for emergency financial
services, making service referrals, approving financial assistance
payments, and data entry. The EFS Program covers the Tri-County
Region (Putnam, Rockland and Westchester Counties), as well as the
five boroughs of New York City.Case finding activities. Includes
outreach to settings where target population is known to reside or
congregate, as well as in reach to eligible clients within the HVCS
organization.Complete screening and intake assessment to collect
demographic, HIV-related information, and information documenting
necessity for emergency financial services.Make appropriate
referrals for clients so they may access needed services including
health, legal, mental health, medical and non-medical case
management, housing and substance use.Completes documentation in a
clear and comprehensive manner which is in compliance with agency
standards and requirements.Case finding activities (Inreach and
Outreach).Employ skill techniques to actively support consumers in
activities that improve their health outcomes resulting in
sustained viral load suppression.Complete intake assessments with
consumers and collect the necessary documentation to support their
need for assistance.Address and remove barriers to enable consumer
access to all necessary components of health care, including mental
health and substance abuse services.Coordinate services with
internal programs and outside community agencies and provide
information and referrals for clients and community service
providers.Negotiate and advocate on behalf of consumers for
services that support self-sufficiency and self-management;Complete
documentation in a clear and comprehensive manner which is in
compliance with agency standards and requirements.Participate in
training and program development activities.Participate in
department and agency meetings and regularly scheduled supervision
sessions.Monitor program deliverables and assist with preparation
of monthly reports.Maintain client charts within program
standards.Attend and participate in monthly department, All Staff
and other required meetings.Be familiar with Cornerstone policies
and procedures and the Employee Handbook.Maintain confidentiality
of all aspects of Cornerstone including, but not limited to,
patient confidentiality, financials, and employee relations.Perform
other related duties as assigned.Requirements:Associate's Degree in
Human Services or related degree with two years of
experience/knowledge in one of the following areas: case
management, chronic illness, chemical dependency, mental health,
and/or human services; or, High School Diploma with five years of
experience/knowledge in the previously mentioned areas.Familiarity
with regional HIV primary care, mental health, substance abuse, and
other services and resources; Understanding of community level work
and the importance of collaborating and coordinating with other
organizations is needed. Strong presentation, communication and
documentation skills, bilingual (English/Spanish)
preferred.Travel/Time Requirement: Must have a valid, unrestricted
driver's license and independent means of transportation for
frequent travel to locations throughout the assigned county as well
as to the offices of Cornerstone. Occasional travel between
