Job Description:
Comunilife's Life is Precious (LIP) program is expanding to
Poughkeepsie and Yonkers, New York. The Case Manager is responsible
for providing program coverage, and to provide individual, group
and family supportive counseling to the youth and their families.
The Case Manager will lead and/or assist with activities and groups
in collaboration with other staff; and two - to increase referrals
to the program by providing Outreach and Awareness workshops and
presentations to the broader community, and by developing,
nurturing and strengthening community partnerships with schools,
mental health clinics, youth programs, and other stakeholders. This
position is eligible to participate in our generous benefits and
paid time off package and the salary ranges from $45,000 to $50,000
a year.Essential Functions ? Provide holistic case management and
counseling to connect the participants and their families with
services, and support.? Provide outreach, engagement and follow-up
with participants referred to the program to assure continuity of
mental health treatment and encourage participation in the Life is
Precious---program. Assist with participant recruitment.? Assist in
the creation of age-appropriate, positive youth development
programming for teens that serve to expand horizons and provide
healing, calming experiences for the clients.? Reinforce academic
and vocational exploration, internships and community service to
enhance participants' career planning, learning, personal growth
and civic responsibility.? Coordinate trips, celebrations and
special events for young adults and their families thatincrease the
clients' cultural pride and awareness.? Collaborate, and develop
partnerships with community agencies, mental health providers,
schools, local organizations, and other stakeholders. ? Participate
in program team meetings to discuss accomplishments, High Risk Case
Reviews,and program related matters.? Represent the program to
community providers (i.e. schools, churches and communitycenters)
and provide information and training relevant to the program focus
and populations.? Document community activities, outings and
development involving families.? Maintain case management
documentation such as progress notes in AWARDS, with special
attention to Hospitalization and Incident Reporting; support and
encourage staff cohesion; promote LIP mission and values and work
to staff and community, create and share group curriculums for
Youth Development Groups.Education and Experience Requirement(s)BA
or BS in human services field; experience working with children and
