SUMMARY The CHHA Clinical Supervisor will be responsible for the clinical supervision, guidance, and mentoring of the clinical program staff in order to ensure the delivery of quality home care services to patients served. ATTRIBUTES BSN or ASN with a minimum of 2 years Home Care experience. Clinical expertise through experience. Certifications in clinical topics and/or leadership preferred. Computer proficient. Travel required. RESPONSIBILITIES Leadership. Provides leadership and direct supervision of assigned clinical staff to include but not limited to: co-visits with staff, OASIS supervisory visits and competency/clinical supervisory visits per policy, documentation oversight, case management and case conferencing. Case Management. Case management of patients, documentation in NetSmart required. Onboarding. Participates in the professional staff orientation process as assigned. Provides professional staff with education/mentoring in regards to work performance, performance improvement, in-service and skill development. Participates in interview and on-boarding processes for new staff. Scheduling. Collaborates with schedulers on assigning patients/caseloads. Training. Participates and supports Home Health Aide training. Quality Control. Responsible for chart audits and quality planning. Flexibility. Participates in the supervisory on-call rotation. EDUCATION AS: Nursing (Required) LICENSES/CERTIFICATIONS RN - Registered Nurse - New York State Education Department (NYSED); BLS - Basic Life Support - American Heart Association (AHA) Rochester Regional Health is an Equal Opportunity / Affirmative Action Employer.
