Requirements:** Graduated from an accredited Physical Therapy (PT)
college or university.* Unencumbered Physical Therapy (PT) license
for New York.* Conduct home visit Physical Therapy assessments of
referred patients providing direct patient care to assess medical
conditions, functional capabilities, limitations, and potential for
rehabilitation.* Proficient in EMR documentation (we use Hello Note
but experience in this system is not required).* CPR
Certification.* Full time and part time roles available*Physical
