Job Description:

Saint Joseph's Medical Center is seeking a Full Time Continuing Care Coordinator for our Case Management Department with demonstrated skills in core and primary care specific competencies based on the patient population served. Responsible for cross continuing care coordination, work with physicians, peers, and community-based organizations and insurers, facilitating safe discharge plans for all patients.Education and Qualification Requirements

Graduate from an accredited school of nursing.
BSN Preferred
Licensed in NY State
BLS Required
