Job Description Job Description Role Summary Our client, a home health care provider serving the greater Philadelphia region, is seeking an experienced and forward-thinking Patient Care Manager to oversee and enhance the delivery of patient care services. This leadership role is responsible for ensuring high-quality, compliant, and patient-centered care while supporting and developing clinical staff within a regulated home health environment. Key Responsibilities Lead and manage patient care operations to ensure compliance with all applicable regulatory and professional standards. Supervise, mentor, and support nursing staff, promoting professional development and strong clinical outcomes. Collaborate with interdisciplinary teams to develop and implement individualized patient care plans. Ensure accurate, complete, and timely clinical documentation. Conduct regular audits of patient records and care plans to support quality assurance and survey readiness. Serve as a liaison between patients, families, clinicians, and healthcare providers to support coordinated care. Participate in continuous quality improvement initiatives and ongoing professional development activities. Required Qualifications Prior experience in a home health or home care setting. At least two (2) years of management or supervisory experience in a clinical environment (preferred). Strong knowledge of accepted professional nursing standards and practices. Working knowledge of Medicare Conditions of Participation and applicable federal, state, and local regulatory requirements. Demonstrated leadership, communication, and organizational skills. Licensure & Certification Requirements Active, unrestricted Registered Nurse (RN) license in the state of practice. Compensation & Work Model Full-time leadership role within home health operations.
