since 1981. We are seeking a Full time RN FIELD SUPERVISOR who will
be based at our Yonkers location. As a field supervisor, you will
further Bestcare’s goal for our patients to function safely, at
their optimum level of independence. Through education,
communication, empowerment, and technology we are focused on
transformative care while optimizing patient and employee
satisfaction. Job Responsibilities: · Conduct initial home visits
to assess patient needs and develop the Aide Plan of Care ·
Communicate with a patient’s physician and other team members to
ensure delivery of appropriate patient care · Supervise patient
care provided by our Home Health and Personal Care Aides ·
Participate in orientation, teaching, and evaluations of care
providers · Responsible for the review and maintenance of patient
records, using telehealth modalities for early intervention of
patient issues Job Qualifications: · Must be a valid Registered
Nurse licensed in New York State, preferably with two (2) years of
experience · Must have a valid driver’s license and transportation.
· Previous home care experience is a plus · Bilingual Spanish and
