Summary: Ref : SM-U-1611 Specialty: LPN - Long Term Care Job Type:
Contract/ Per Diem Location: Salem, NJ Shift Type: Day, Evening,
Night (8, 10, or 12 hours) Rate: 34.4 Requirements: Must hold
active LPN/LVN license in practicing state. LTC/SNF/Assisted Living
experience preferred. Strong clinical, communication, and care
skills. Able to follow care plans and collaborate with staff.
Responsibilities: Clinical Care: Assist with ADLs and monitor
resident health status. Administer medications and treatments.
Document resident care and report all changes. Collaboration &
Safety: Follow physician orders and established care plans. Promote
a safe, clean, and supportive environment. Provide emotional
