Admissions Director -Full Time Day Shift

The Admissions Director will assist in assessing children who
would match the children’s center’s admission criteria to maintain
census at established budgetary standards, and provide case
management for medically fragile children. In addition, understands
ESCC Mission and Core Values and incorporates values into daily

Position Functions and Responsibilities
Promotes Elizabeth Seton Pediatric Center’s Mission and Core

Relationships by bringing these values to life in the
Liaise with the Insurance Plans/Managed Care Case Managers,
providing clinical updates, obtaining authorizations and other
information as needed for continued stay; work with
interdisciplinary team to ensure treatment plan goals are being met
and documented accordingly.
Develop working knowledge of the admissions procedures.
Collaborate with the interdisciplinary team to review referrals,
to determine which referrals meet the criteria for admissions.
Make onsite/in-hospital assessments of prospective admissions as
part of the admissions decision-making process.
Liaise with external referral sources during the admission
process (gathering clinical data, presenting cases to the
Admissions Committee, providing information about ESCC and touring
families through the facility).
Process each admission decisively with regard for
confidentiality of all information related to: verification of
insurance, referral source documents, medical information.
Collaborate with interdisciplinary team in making neighborhood
and room assignments, and coordinate transfer meetings.
Maintain confidential data base on referrals to admissions data;
populate the Daily Grid of active referrals; and update the census
and bed board.
Coordinate with ESCC medical team and with external hospital
discharge planners to plan for resident’s readmission to ESCC from
Review PRI, Screen & OPWDD Level 2 documentation prior to any
admission or readmission.
Share all clinical information received via all scripts and/or
fax with ESCC interdisciplinary team for admission and/or
Required Skills Education Requirements:
Licensed RN in New York State Preferred
Required Experience Experience:
Care management or case management experience in a Health Plan,
Hospital, Clinic or Nursing Agency, with five year experience.
Demonstrated communications skills with ability to effectively
present at all levels.
Bilingual in Spanish preferred.
